Arrange a visit that has a point

Use the form to tell us who needs to be seen, which department fits, and the day you hope for. A representative reviews requests and confirms the details with you. This page does not hold an emergency line. If symptoms feel dangerous now, go to the nearest emergency service rather than waiting on a form.

Same-week openings are kept for new problems that are uncomfortable but stable: a spreading rash, a persistent fever you can manage at home, a joint that has swollen, a child whose cough is wearing the household down. Routine reviews and paperwork visits are booked a little further out so urgent stories are not stuck behind them.

Before you come

  • List medicines, doses, and the supplements you actually swallow.
  • Note when the problem started and what makes it better or worse.
  • Bring glasses, hearing aids, and previous results you already have.
  • For fasting blood work, take water unless you were told otherwise, and eat as soon as the sample is done.
  • If a child is the patient, pack a snack for afterward and tell them the truth in small words.
